Architectural Request for Change Form

All submissions must be in compliance with any existing HOA policy. This pertains to FENCES as well! Please check the HOA Documents section, here or your owner portal to see if your community has specific guidelines for your request. This submission is not a guarantee of approval. Thank you.

For Ground Altering Projects: IE.: fence, shed, patio, concrete pad, flower bed, in-ground flag poles, trees, etc.

You must include:
1. A plat map of your property showing the exact placement of the changes. A plat map is a professional rendering of your property, from a bird's eye view, showing the property lines and easements. This can be obtained from the documents you received from your closing, or you can usually get a free copy from your city's office. Please click here to see sample images of a plat map.
  - For Fences: Details on plat must include location of gate(s), measurements including height, width and length of fence.
2. Your city office may have documentation you need to fill out and get approved. Please include any approved documents from your city office. For example certain cities have documents they require for approval of fences or pools.
3. Sample image of finished project. For a fence your contractor may be able to provide a sample image of the fence or if a neighbor has the exact same fence, you can snap a picture of their fence.
4. Pictures of area before any changes have been made (snap a few cell pics of your yard. If for a fence please make sure to include the property lines).
5. Please include detailed information for the project, ie: material used (metal, stone, vinyl), color (final finish color- Example SW 7645 Thunder Grey by Sherwin Williams), all measurements.
For Non-Ground Altering Projects: ie, door alteration, flagpole attached to home, shutters, windows, satellite dish, etc.:

1. Pictures of area before any changes have been made (snap a few cell pics of the area where you are making changes).
2. Sample image of finished project. For example: Adding a storm door, if purchasing the storm door online you can provide the online image of the door.
3. Please include detailed information for the project, ie: material used (metal, stone, vinyl), color (final finish color- Example SW 7645 Thunder Grey by Sherwin Williams), measurements.
IMPORTANT - Any submission made without all of the required information above will be denied and you will be charged for any re-submissions (if submission fees apply to your community). Please have any approved permits, sketches, plat map, detailed information for your project ready before completing this form.

I understand that under the Homeowner Association’s Master Deed, Bylaws, and Amended and Restated Rules and Regulations, the Board of Directors, ARC Committee or Developer(s) will act on this request and provide me with a written response of their decision. 1. No work or commitment of work will be made by me until I have received written approval from the Board.
2. All work will be done at my expense and all future upkeep will remain at my expense.
3. This includes the replacement of any parts or materials used in conjunction with this alteration.
4. All work will be done expeditiously once commenced and will be done in a professional manner by myself or a contractor.
5. All work will be performed at a time and in a manner to minimize interference and inconvenience to other unit owners.
6. I assume all liability and will be responsible for all damage and/or injury which may result from performance of this work.
7. I will be responsible for the conduct of all persons, agents, contractors, and employees who are connected with this work.
8. I will be responsible for complying with, and will comply with, all applicable federal, state, and local laws, codes, regulations, and requirements in connection with this work, and I will obtain any necessary governmental permits and approvals for the work. I understand and agree that the Board of Directors for the Homeowners Association, and its agent, have no responsibility with respect to such compliance and that the Board of Directors approval of this request shall not be understood as the making of any representation or warranty that the plans, specifications, or work comply with any law, code, regulations, or governmental requirement.
9. I understand that a decision by the Board of Directors will be made at a Board Meeting.

1. I understand that if I sell my condominium/townhome/home I am responsible for notifying the next homeowner of any approved architectural change, and informing the next homeowner of their responsibility for all future upkeep.
2. I understand that any work not started within ninety (90) days from the approval date that I may need to submit a new ARC form for approval due to the delay in starting on the project.
3. I fully understand that I will be responsible for any and all damaged caused by the installation of any item upon my lot or home both present and future. Further in the event that I have installed an item on a common element such as a door frame, roof structure, fence or deck or any other item listed as a common element in the CCR documents for my community that I will be responsible for the removal and replacement of the item in the event that any maintenance or replacement should be done to my unit both scheduled and unscheduled. I fully am aware that there may be an additional cost for the maintenance work as a result of the installation of my item and that I will pay for all of the charges associated with said work. In the event that the item I am installing is a life safety item such as but not limited to a hand rail I will be responsible for the maintenance of all components of this hand rail. This includes but is not limited to the hand rail surface, paint, rust, lateral support, stabilization members and further release and agree to hold harmless the home owners association, board and management company for any injury that may result of my lack of maintenance to this item or any and all failures of the item. I fully understand that no inspection for the life safety item shall be conducted by the homeowner’s association, board or management company. The ownership of the installed item shall remain that of the owner and not the homeowner’s association or Management Company even though it has been installed in or on the common area. In the event of wood rot or any and all detrition of any type of surface or area that the item has been installed on this includes but is not limited to wood, siding sheathing, trim or any and all components of the building that I shall be responsible for any and all repair or replacement cost associated with such detrition.
4. In the event that any alterations are made from the approval, all work will need to be stopped and a new submission will need to be completed.
5. No work shall start until such time as I have received the document back fully executed.
6. In the event that this request is for a dish placement the following shall apply, I fully understand that any and all damage that is caused to any structure, component, building material, roof shingle, wiring, plumbing, gutter, trim, or siding whatsoever from the installation of this dish or any of its components shall be the responsibility of the unit owner present and future. I understand that in the event that I sell my property I will inform the new buyer of this obligation in writing failure to do so shall not relive my obligation to the association only once I have notified any buyer in writing shall my obligation be passed to the new buyer. In the event the dish falls and strikes a person or property owned by any entity or person other than the owner of the dish and causes damage dish owner agrees to hold harmless and indemnify the homeowners association for any and all damage to a person or persons as well as any property that may be damaged owner agrees to promptly settle any cost with the effected party. In the event that the dish becomes inoperable it shall be the owner’s responsibility to remove the dish. In the event that the owner does not remove the inoperable dish within 30 days of the dish becoming inoperable the association may at its option remove the dish and place any and all cost of removal and repair on to the owner account any said billing shall be promptly paid with or without demand by the association. Such billing shall be subject to any and all late fees as well as collections policy. No wires connecting the dish shall be visible over a span of 8Ft. No wires shall be permitted to run under or in a gutter or along any exterior siding or facia. All dish wire installation shall be concealed from view.
1. All submission processing times and dates shall be Monday to Friday from 9:00 AM to 5:00 PM excluding any and all federally designated holidays. i.e. If you submit your form on a Saturday, it will not be seen or processed till the dates listed. This does NOT mean it will be approved during this time, this means it will begin the review process!
2. All submissions may take up to 30 days to be processed and reviewed. There may be delays that are out of the control of the management team as the review process is done by the community board/developer. This submission is not an approval, it is only a request. No guarantees of approval are offered within this submission.
3. Submissions will not be processed until ownership of the property is noted in the management software. In the event that you are submitting this request prior to ownership of the property the request may be held until such time as your ownership is entered into the system.
